Post 1 – 37 hours, temporary to March 2018, Complex Care Team

Post 2 – 18 hours, temporary to March 2018, Complex Care Team

Post 3 – 37 hours, temporary to August 2019, Primary Care Wellbeing Team

 

Please note that Post 3 is working within the Primary Care wellbeing team and is predominantly working with people with mental health issues.

 

Our Integrated Care Team provide a dedicated and trusted point of contact for individuals, carers and families to assist with the navigation of their care and support planning; providing advocacy on their behalf within a multi-disciplinary team and developing innovative ways of supporting people with complex care needs, to remain independent for longer and experience better health and wellbeing.

 

The Personal Support Navigator will ‘bridge the gap’ between the health and social care systems and individuals.

 

You will have experience of working in a community setting, preferably within the voluntary sector and be committed to using a person centred approach to ensure the personal aims and choices of people are properly represented within the multi-disciplinary team. You should have the ability to think innovatively and find creative solutions; involving other organisations and volunteers; being effective as a facilitator rather than a fixer.

 

The nature of the post will require travel across the District. A full driving license, appropriate insurance cover and use of a car is essential.

 

Salary £23,000 per annum (pro rata for part time post)

Hours worked on a flexible rota Monday – Friday. Occasional evening or weekend work may be required.

5 weeks holiday plus bank holidays (pro rata for part time post)
