Transition Service Manager (Preparing for Adulthood)

We are looking for a highly skilled and motivated professional to join an Adult Social Care Management Team, providing leadership and management to a developing Transition Team.
The Transition Team is a service which supports young people and their carers from the ages of 14 – 25 years. The team is based within Adult Social Care, and has been set up to work alongside key agencies to help young people prepare and move into adulthood. The focus of this position will be on ensuring a high quality of service to promote a personalised and universal social care support system to vulnerable members of society, maximising well being and independence in the local community.

You will be responsible and accountable for the day-to-day delivery of high quality social care support to the young people preparing for adulthood.

You will be an integral member of the management team and will have the opportunity to promote consistent and innovative best practice across the District.
- You will have responsibility for a delegated budget and for improving and maintaining high performance standards of the service you are responsible for.
A significant focus of your work will be to work strategically with partners from the Children's Service, NHS, schools and colleges, care providers and the independent and voluntary sector. You will develop effective relationships that are focussed on improving the outcomes for younger people moving into adult services. You will also be involved in the Special Educational Needs and Disabilities (SEND) working alongside colleagues from children's services.
We are looking for someone who:

Is enthusiastic and innovative.

Is able to provide strong leadership for the local social care teams they will be responsible for.

Holds a professional qualification in social work and a proven track record of successfully managing in a social care setting.

Is able to demonstrate a high level understanding and commitment to the personalisation agenda and have both direct experience of delivering statutory social care services and a comprehensive and applied knowledge of adult social care legislation.

Has an understanding and commitment to supporting carers and demonstrable commitment to delivering support and managing staff in accordance with the Councils Equality and Diversity policy.

Holds a current full driving licence (unless a disability prevents this).
