Community Care Officers x 4 (SCP 17 to 25 with bar at SCP 22) £26,845 pa - £32,020 pa (pro rata for part time posts)

 

A number of permanent full-time and part time posts available:

 

2 x 37-hour posts and 1 x 30 hour post within Airedale General Hospital and Bradford Royal Infirmary Social Work Teams working across Acute, Intermediate Care and Discharge to Assess beds. (Hybrid)

 

1 x 37-hour posts in the Independence Advice Hub. Front Door Duty Team

Bradford Short Term Support Teams within IAH and Hospitals are dynamic and continually evolving, working with a strength-based approach to support people within the community and those being discharged from hospital. We work to the Home First principle in a person centred and strength-based way to enable people to remain independent and integrated within their community under the ethos Happy, Healthy and at Home. We recognise the importance of carers within the district and value their contribution and need for support in their own right.

 

In Bradford, we are fortunate to have our own in-house enablement services which provide packages of care and short-term beds. These enable ongoing assessments outside of the acute hospitals in line with the National Discharge to Assess guidelines. These services are also accessed via IAH from those living in the community. We have excellent working relationships with Health and Voluntary Sector Partners and work in an integrated way to support hospital discharges and promote enablement. 

 

As part of this role, you will be expected to take part in Early Help and Prevention duty work, assessment, short term case management, working in a collaboratively way with professionals, colleagues and other services. We welcome applications from current Community Care Officers who may hold an NVQ or equivalent in Social Care as well as applicants with the relevant experience and skills. You will be working with integrated multidisciplinary teams alongside health colleagues and qualified Social Workers.

 

 

You must have:

 

  • Experience in adult services, working with vulnerable adults and their carers’ and undertaking assessment and support planning. This Includes assessing risks and emotional resilience.
  • An understanding what harm or abuse is, recognising it and responding to it proportionately.
  • A commitment to working in partnership with others within the service and external partners to put adults and carers at the heart of decision making.
  • Good communication skills and be able to share appropriate, succinct, objective information and analysis to aid joint decision making.
  • Have a full, valid driving licence and car available for work (unless a disability prevents this) or are working towards achieving this.
